We understand it can be difficult to fill out forms in the waiting room.
The simplest way for us to gather the required information is for you to complete these forms online.
Please complete both the New Patient Form and the HIPAA Notice.
Otherwise, you can be old-school, and download and complete each form before your first visit. Bring them with you or fax them to us at 801.766.5341 (we can be old-school too).
